How Cigna works at this practice

We are in network with Cigna, including plans administered under Evernorth, Cigna's health-services arm.

Cigna plans vary widely between employer groups, and the variable that matters most here is usually your deductible rather than the network. Early in the calendar year, before the deductible is met, a covered procedure can still carry real cost; later in the year the same procedure can cost you very little. If timing is flexible, it is worth knowing where you stand before scheduling, and we will tell you.

Like most commercial carriers, Cigna generally requires prior authorisation for radiofrequency ablation and nerve stimulation. We submit it.

What we bill to Cigna

Under an in-network plan we bill your evaluation and the interventional work that follows it: epidural injections, medial branch and nerve blocks, sacroiliac and joint injections, radiofrequency ablation, and spinal cord and peripheral nerve stimulation. All of it is placed under live X-ray or ultrasound guidance rather than by landmark.

The conditions most often treated under this coverage: back pain, sciatica, neck pain, herniated disc, spinal stenosis, knee pain, shoulder pain, hip pain and peripheral neuropathy.

What is not billed to insurance

Two things are not billed to any plan, this one included. Regenerative treatments — platelet-rich plasma and bone marrow concentrate — are not covered by United States insurers and are quoted directly. SoftWave and MLS laser therapy are likewise usually self-pay. You are told which category your treatment falls into before it is scheduled, not after. The full picture is on insurance & cost.

  • Your Cigna card and a photo ID.
  • A referral, if your plan requires one. Call us if you are not sure.
  • Any imaging relating to the pain — MRI, CT or X-ray.
  • Notes from prior treatment: physical therapy, injections, surgery.
  • A list of your current medications, including doses.

It depends mostly on how much of your deductible you have met for the year. Once your benefits are verified the office can give you an estimate for the specific procedure being planned, before you commit.
Being in network — does that mean my treatment is covered?
Not by itself. In network means we have a contract with the carrier and you are seen at your in-network benefit level. Whether a specific procedure is covered, and what you would owe, depends on your plan and sometimes on prior authorisation. The office verifies your benefits and gives you an estimate before anything is scheduled.
