Sudden loss of bladder or bowel control, numbness in the groin area, or rapidly progressing weakness in the legs needs emergency assessment, not an appointment request.

If you are in crisis or thinking about harming yourself, call or text 988.

Appointment requests and coverage estimates

Submitting a request through this website is a request, not a confirmed appointment. Your appointment is confirmed when our office confirms it with you directly.

If you use the insurance coverage check, the result is an estimate based on what your plan reports to our clearinghouse at that moment. It is not a guarantee of coverage or of payment. Your actual responsibility depends on your plan, your deductible at the time of service, the specific procedure performed, and your plan's determination of medical necessity. Prior authorisation may still be required.

Your responsibilities

Give us accurate information. Verification and scheduling depend on it.
Do not submit anyone else's personal or insurance information without their authority.
Do not send detailed medical records, images or documents through this site or by ordinary email — neither is a secure channel.
Do not attempt to disrupt the site, access it by automated means at a rate that burdens it, or use it for any unlawful purpose.
